Permanent Full-Time

St. Joseph’s Hospice of Sarnia Lambton is a faith-based organization that provides quality of life at the end of life, offering compassionate holistic care and support to individuals living with terminal illness. Support is extended to their family members and caregivers, and to those grieving the loss of a loved one. Hospice programs and services are offered at no cost to those we support and are available irrespective of age, gender, religion, race, ethnicity or economic status.

Whether in the community, our Wellness Centre or our 10 bed residence, St. Joseph’s Hospice provides specialized support programs and services with a person centered approach. Individuals and families are intimately involved in creating their own care plan which addresses their unique needs. Our competent and committed Hospice staff and volunteers work together as a team with other community partners and service providers to provide full support and quality hospice palliative care.

St. Joseph’s Hospice is currently seeking a qualified individual to fill the position of Program Manager. The Program Manager will lead the planning, implementation, evaluation and administration of current programs and services. These programs include Caring Hearts Children’s Program, Adult Bereavement Program, Illness Support Program, and Complimentary Therapies. In addition, the Program Manager will be responsible for developing new programs and services to meet the needs of the community of Sarnia Lambton in accordance with the Mission, Vision and Values of the Organization.

Duties and Responsibilities:

• Manage the planning and development of curriculum training modules for program volunteers

• Develop and implement policies and procedures, and monitor compliance to industry standards

• Perform improvement outcomes, manage adverse events and complaints

• Accountable to ensure that direct reports are guided by the standards of professional conduct for their discipline

• Provision of critical incident debriefing and monitor occupational risk/ stress and support for direct reports. Engage in relief for the program team with crisis phone call interventions as needed

• Collaboratively establish and examine fiscal program budgets and manage financial resources for all programs

• Under leadership and direction from the Executive Director, responsible for implementing accreditation standards for community support programming within the Resource Centre, identify accreditation gaps and collaborate with the hospice team toward accreditation

• Research and distinguish current Hospice Palliative Care trends, information and education

• Perform statistical reports through collecting, monitoring data input and generating monthly reports for the Executive Director

• Appraise program monthly calendars, brochures and program website information

• Establish program promotion in collaboration with hospice management team and evaluate impact of various methods used

• Plan and coordinate education initiatives for community and health professionals on various topics related to dying and bereavement

• Collaborate with Fund Development Manager for grant proposals, monitoring program grant requirements and verifying outcome measures

• Perform and analyze Chart Audits for compliance, evaluating trends and implementing practices and changes, and compliance of records management

• Produce and evaluate program satisfaction surveys and identify areas of concern, implementing required program changes based on client feedback

Qualifications:

The selected candidate will have a unique skill set which includes but not limited to the following:

• Master’s Degree in Social work or equivalent with a minimum of 5 years’ experience in a related role, including management experience.

• Member in good standing with the Ontario College of Social Workers and Social Service Workers, or equivalent.

• Certificate in Grief and Bereavement or Thanatology from a recognized post-secondary institution preferred.

• Evidence of training in current grief and bereavement theory and issues related to trauma, and demonstrated understanding of complex grief and complex family situations and concurrent stressors that may be present.

• Experience in program management, development, growth and implementation with demonstrated experience working with both adults and children.

• Experience with assessing, developing and working with volunteers for program delivery.

• Experience working with and sound knowledge of the larger health system and social service network, and the ability to establish and maintain community partnerships.

• Experience with quality assurance initiatives and experience with the accreditation process will be an asset.

• Demonstrated leadership and management skills consistent with the Mission, Vison and Values of St. Joseph’s Hospice.
